Other family support services are directed at
parents, siblings, and other relatives to assist
in enhancing a family’s capacity to continue
caring for a family member with a developmental
disability living at home. Examples include:
Recreation
Planned programs of social, recreational and
leisure activities for people with developmental
disabilities that include opportunities to interact
and participate as part of the community.
Family Reimbursement
Provides for the possibility of purchasing services
and goods that are not funded through any other
sources as long as prior approval has been obtained
and the goods and services have been deemed appropriate
to meet the needs of the person being served.
Traumatic Brain Injury
AIM offers a variety of services that are meant
to provide direct assistance to persons with brain
injury and their families. Our services are funded
through the New York State Department of Health’s
(DOH) Home and Community Based Services (HCBS)
Waiver program.
Eligibility Criteria
for HCBS Waiver Services
The Waiver Applicant:
Must be a Medicaid recipient.
Diagnosed with Traumatic Brain Injury or other
type of brain injury, such as stroke, anoxia,
etc.
Individuals injured between the age of 18 - 22
should seek services from OMRDD before DOH.
Must be eligible for a nursing facility level
of care (determined byPRI/Screen).
Must choose to live in the community rather than
in a nursing facility.
Can identify a safe living arrangement that will
meet their needs.
Can be supported safely within the community with
the funds and services available under the HCBS/TBI
Waiver.
Services Available through DOH/HCBS Waiver Program:
In addition to services available through Medicaid,
individuals determined eligible for the Waiver
Program may also develop an Individualized Service
Plan including any of the services listed below.
Independent Living
Skills Training
Provides one-to-one support to focus on increasing
independence in practical skills, such as cooking,
shopping, banking, etc.
Home and Community Support Services
Provides guidance and assistance to enable the
Waiver participant to reside safely within the
home and community environments.
Environmental Modification
Provides adaptations to residences to promote
independent and to ensure safety.
Special Medical Equipment & Supplies
Provides durable and non-durable equipment not
funded under state plan Medicaid.
