Community Habilitation for People with Disabilities

This service is goal-driven and meant to enhance your or your loved one’s independence through building skills based on personal interests and needs, and encouraging involvement in the community. It is typically a one-on-one service, but can be delivered as a group. This service is more individualized compared to the group-based setting of Day Habilitation

Goals can include, but are not limited to:

  • Increasing independence
  • Learning to cook
  • Budgeting
  • Learning local resources
  • Getting a driver’s license
  • Building social capital in the community
  • Travel safety

This service can be delivered to people living at home in the community, as well as those living in Residential Settings.


To be eligible for Community Habilitation, you or your loved one must;

  • Have a Care Coordinator.
  • Be Waiver Services enrolled (Your Care Coordinator will assist in enrollment).
