Supported Decision-Making Agreements (SDMA) provide people with intellectual and developmental disabilities [Decision Makers] the legal capacity to make decisions about their lives with support from people they choose [Supporters]. Through facilitated Supported Decision-Making, decisions related to healthcare, finances, housing, employment, and more, are explored. Decision Makers choose trusted Supporters to help them with decisions that are important to them, and those Supporters agree to provide that support.
Supported Decision-Making facilitation is free to Decision-Makers and families through a funded grant provided by OPWDD.
Who Is Eligible?
To participate in facilitated SDM:
- You must meet OPWDD eligibility requirements, as Supported Decision-Making facilitation is provided at no cost through funding from OPWDD.
- Be at least 17 years old
- Want to participate in facilitation to be your own Decision Maker
How Does It Work?
Decision Makers work with a trained Facilitator and Mentor to build a support network and create a legally recognized Supported Decision-Making Agreement (SDMA). This agreement provides the legal capacity to make decisions with support.
The facilitation process is unique to each Decision Maker and can typically take around 12 months to complete. Facilitation can occur in person, remote or a combination of both. Agreements can be updated at any time as needs and Supporters can change.
