Traumatic Brain Injury/Nursing Home Transition Diversion Waiver
AIM offers a variety of services that are meant to provide direct assistance to persons with brain injury and their families. Our services are funded through the New York State Department of Health’s (DOH) Home and Community Based Services (HCBS) Waiver program. The Service Coordinator assists individuals with access to necessary supports.
Do you know someone who has sustained a Traumatic Brain Injury? Do you know someone who may be eligible for a Nursing Home but is looking to remain living in the Community? AIM Services provides Service Coordination through the TBI Waiver (Traumatic Brain Injury) and the NHTD Waiver (Nursing Home Transition Diversion). We provide services in Saratoga, Warren, Washington, Schenectady, Rensselaer and Albany Counties.
We have Service Coordinators who can provide linkage to related services in the community including advocacy. These services are person-centered, individualized, and funded through Medicaid.
