Before you begin, please be sure to have the following information ready:
- Tabs number (if you receive services from OPWDD)
- Your OPWDD Eligibility Letter
- Care Manager contact information
- Self Direction Fiscal Intermediary information (if you receive Self-Direction services)
- Your most current Life Plan
Participating in the Supported Decision-Making (SDM) process and completing a Supported Decision-Making Agreement (SDMA) will not affect an existing guardianship.
