Can I participate if I already have a legal Guardian?
- Yes. You can participate in the informal process and even sign a Supported Decision-Making Agreement (SDMA), but it won’t be legally recognized while you have a Guardian.
Is there an age requirement?
- Yes. You must be at least 17 years old, have an Intellectual/Developmental Disability (I/DD), and are eligible for services through the Office for People with Developmental Disabilities (OPWDD).
What will I gain from Supported Decision-Making?
- You will build decision-making skills for now and the future
- Your supporters will learn how to help you make choices your way
- With a Formal Agreement, you’ll have a legally recognized SDMA to use with doctors, banks, and more
How long does it take and what does it cost?
- Typically 6–18 months (you set the pace and can pause anytime)
- No cost! SDM is funded through an OPWDD grant for the next 5 years
How will my information be used?
- Your information is used to provide support, communicate with other necessary parties (with your permission), and meet OPWDD grant requirements.
- We may use documents to train Facilitators and Mentors. Any training use will remove or change personal details to protect your identity.

What if I have feedback or a complaint?
- If you are not happy with any aspect of the SDM process, you can share that with your Facilitator, Mentor, AIM Services or the IDDO program. Please see the Rights and Responsibilities packet you received upon intake for more contact information.
